Our team consists of trained healthcare professionals – including registered nurses, licensed practical nurses, therapists, home health aides and medical social workers . Our non-licensed team consist of personal care attendant and caregivers. Our teams are dedicated to meet your unique health needs through both medical and supportive services:
Skilled nursing care: Registered nurses (RNs) or licensed practical nurses (LPNs) provide medical services such as wound care, administering medications, monitoring vital signs, managing chronic conditions, and coordinating care with physicians.
Rehabilitation services: Physical therapists, occupational therapists, and speech therapists may provide rehabilitation services to help individuals regain or improve their mobility, strength, coordination, speech, or daily living skills.
Medication management: Ensuring clients take their medications as prescribed, monitoring for any adverse reactions or interactions, and providing education on medication usage.
Medical equipment and supplies:Coordinating the provision of necessary medical equipment and supplies as well as ensuring that they are properly set up and education/training on how to use it is provided.
Disease management: Assisting individuals with the management of chronic conditions such as diabetes, heart disease, or respiratory disorders, including education on self-care, monitoring symptoms, and implementing necessary lifestyle modifications.
Home health aides: These certified nursing assistants assist with bathing, grooming and dressing.
Medical social services: Providing counseling and support services to address emotional, social, and financial concerns related to the client’s health condition.
Non-Skilled Services – Personal Care Attendant and Caregivers provide clients/ patient with more thorough care as they assist them with housework, cooking, and errands.
