Dementia and 91 years old.

Is the travel imperative? Under those circumstances, I'd try to find another option. If not, I'd consider that the degree of their progression.   It would be my primary concern, regardless of the method of transportation. Are they incontinent? This can prove quite uncomfortable and challenging for the caretaker and anyone who is around. Also, the person may become quite frightened and disoriented, resulting in alarming behavior. Medial air transport might be the best option, but, that can be expensive.

Is someone going with them? Are you asking about mode of transportation, like flying vs driving or how to accomplish a particular mode given certain problems?
