Call Carefirst to ask to find you a provider in network then give them a call. You probably will have a co-pay for each SW appt.
You may go through several therapists before you find one you like.
BC usually covers this under your mental health benefit.

Do you mean a therapist? A social worker is not going to be able to help you with what you are going through emotionally with caring for your mom.

Contact the Maricopa county counsel on aging and ask them for help. They can direct you to resources in your area.
