The Participating Communities may contact you directly regarding their services. APFM does not endorse or recommend any provider. It is your sole responsibility to select the appropriate care for yourself or your loved one. We work with both you and the Participating Communities in your search. We do not permit our Advisors to have an ownership interest in Participating Communities.
II. How We Are Paid. We do not charge you any fee – we are paid by the Participating Communities. Some Participating Communities pay us a percentage of the first month's standard rate for the rent and care services you select. We invoice these fees after the senior moves in.
III. When We Tour. APFM tours certain Participating Communities in Washington (typically more in metropolitan areas than in rural areas.) During the 12 month period prior to December 31, 2017, we toured 86.2% of Participating Communities with capacity for 20 or more residents.
IV. No Obligation or Commitment. You have no obligation to use or to continue to use our services. Because you pay no fee to us, you will never need to ask for a refund.
V. Complaints. Please contact our Family Feedback Line at (866) 584-7340 or ConsumerFeedback@aplaceformom.com to report any complaint. Consumers have many avenues to address a dispute with any referral service company, including the right to file a complaint with the Attorney General's office at: Consumer Protection Division, 800 5th Avenue, Ste. 2000, Seattle, 98104 or 800-551-4636.
VI. No Waiver of Your Rights. It all depends on how much money you have. Communities usually mean independent living, Assisted living then longterm care. You usually "buy in" and still pay rent for you accomodations. These communities are not cheap. Medicaid does not usually come into the picture till LTC. The question you need to ask is "Do you take Medicaid".
Usually if in independent living you get no help and if you need it, you pay for it. Assisted living, it depends on your needs. Another question you ask during your interview.
These Communities vary by State. We really cannot tell you what they offer in way of help. This is a question for the Administrator. I would have a family member with me for another set of ears. Don't sign anything until you have a lawyer look over the paperwork.

You asked "what kind a help".... help in what way? Medical? Caregiving? Finance?
The 55+ communities in my area, there are too many to count, usually one has to be self-pay, thus if you rent or buy a condo or house, it is up to you to pay out of pocket the rent/mortgage and everything else one pays in rental/ownership. There will be a Homeowners Association fee every month which pays for the benefits the community offers (whether you use them or not).
Now, there are also low-income 55+ communities, where one pay rent according to their monthly income. These communities are conveniently located close to shopping and medical care.
The first step is touring communities. You do not say where this "Elmhurst" is I imagine just about every state has a village, city they call Elmhurst. Depending on your finances you may want to look beyond a specific city. Once you have "toured" a variety of Continuing Care Communities ask if you can "try it on" for a week or two and see how you like it. And there are a variety of 55+ communities. Some are just houses or manufactured homes that restrict the age of residents some are just apartment or condo buildings and you rent or buy and again the restriction is age. Then there are Continuing Care Communities that have Independent Living, Assisted Living, Memory Care and Skilled Nursing all in one community. Others have just Independent Living, some have Assisted and Memory Care. So figure out what level of services you want and begin your search.
