People can have ups and downs with dementia, but can also experience delirium (temporary state caused by pain, electrolyte imbalance, infection, etc.) and the two can look very similar. With elderly people, UTIs can cause really severe confusion and weakness so it's something to ask the dr about if they don't find a more immediate cause.

Our loved one with dementia was fixated on dates and times, and I've heard the same from other several other people whose family members have dementia. It can be really trying. Also the clinginess is common especially in evening. That's really kind that you made the appointment for her -- poor lady:(
