Should we put her assets and money into a trust instead of a deed?

Really you need to see an elder care attorney. There will be paperwork needed to clearly establish the proper % of ownership not only for mom's possible Medicaid application later on but also to get the proper legal on the property.

I'm going to assume that eventually mom will need a NH and will apply to Medicaid to pay for it - one thing that needs to be done is to have it documented that the son has been able to provide for caregiving for her so that mom can get an exemption for any Medicaid recovery of her assets later on after she dies. The amount of time and documentation needed depends on your state's Medicaid program & the elder attorney will know what works.

Is the property one of those that has been handed down, never going through probate and ownership done by quit claim deed or deeds over the years?
