I had my brothers' take Dad. Let the men in the family handle it. Find your dad a "male" primary care physician. The male docs are familiar with this and know how to handle them.
What happens if you do not do all the things for him that you have been doing. How would he get groceries? Would he clean his house? STOP doing what you have been doing. Wait a week, 2 weeks, 3 weeks. Call APS and tell them that he is not taking care of himself. They will follow up. If they think he needs to be evaluated he will be.
The other option that has been mentioned is to tell this that his Insurance requires that he have an annual physical or they will drop his policy might work.
If you have been doing all these things for him because he can not function on his own this is going to be a bit more complicated than just getting him to the doctor. Why do you think he needs Home Health Care or a "Nursing Home"? Who is legally responsible for him if he can no longer make decisions for himself? You or another family member might have to become his Guardian. if no one wants that responsibility then the Court will appoint one. The Guardian will then make decisions for his welfare, both physical and financial.
Your profile says that you are planning to move away in 6 months. What support will your father have then? None of his ailments seem urgent now, so perhaps you should look at talking to him about emergencies.
