You cannot create one. If her service was handled by a funeral home, you should have been given some. They must have a seal. Not sure if the County keeps death records or the State.

This is difficult to believe.
Have you checked your county records?
Who pronounced your wife's death?
When and where did your wife die, and was she buried from a funeral establishment?

I don't know exactly how to guide you with registering a death but I would check with the county office of records where she died.

"To register the death you must be either:
a relative or next of kin
someone who was present at the death
the person responsible for making funeral arrangements (but not the funeral director)
the owner or occupier of the place of death
the executor (the person/s named by the deceased to carry out the terms of their Will)"

There has to have been a death certificate completed.
Where did your wife die?
Who was her doctor?
Was she cremated of buried?
If she was in the hospital when she died the Hospital completed and filed the death certificate.
If she was taken to a Funeral home and cremated or was buried a Death Certificate would have been needed. Typically the Funeral home makes the arrangements to get you the number of copies you want.
If she was at home on Hospice the Hospice is responsible for filling out the death certificate and the Hospice Dr will sign it.
You could contact your County Vital Records office or your State Department of Public Health Vital Records office.
