We had our financial POA done by a lawyer because that was more likely to face scrutiny but the I just downloaded the medical one and had it witnessed by mom's neighbours (and although we were asked if we had it I'm not sure I ever had to show it)

As per your profile, a person doesn't need to have care experience to be someone's Medical Power of Attorney. No different than a person doesn't need to be a CPA or Accountant to be someone's financial Power of Attorney.

I think that you may be best to google this for your own area. It varies here in the states. Basically it is best done by an attorney, and then all the things that NEED TO BE in the document will be in it.

Much like in the states this will require that the person who is GIVING YOU (conferring upon you) the title of POA must be mentally competent to do so, and will be examined by the attorney as to soundness of mind.

The POA is responsible to act AS DIRECTED by the person conferring the duty upon them. That is to say if this person wishes you to handle finances then that is what you do, and you will keep exacting records of same.

At any point that the principal, who conferred the POA, is unable to act for him or herself you will take over.

There should also be consideration to doing advanced directives and medical POA at the same time.
