Not necessarily Medicaid-based, just person-centered. For instance, a meeting including the person involved, care partner, Elder Services Care manager, Independent Living Counselor, Council on Aging Outreach worker - anyone else? Is there a 'Massachusetts model'?

Kathleen: I'll give you a glimpse into the "Massachusetts model." We tried to make my mother move.Many times! She was a legally blind, 94 y.o. woman with CHF and A-Fib who demanded to live alone in her own home in Massachusetts. Until I showed up after driving 10 hours straight and moved in when her BP ran 60/40.

What the person wants is usually to stay in their home. Unfortunately they reach a point where that is extremely unsafe. At that point, if relatives have not stepped in, the county social worker will. It's very heartbreaking to deny their wishes in order to keep them safe.
