It is for my Grandma who had heart surgery is in diapers and home bound but does not qualify for medicaid - so she says?

Normally, the doctor fills out and signs this form. I know it's used for Medicaid. I didn't realize it's also used for Medicare. The doctor may need help in completing this form, since it asks what the patient needs assistance with. He may not know the answers without input from the patient's family. For example, what are bowel and bladder needs, feeding needs, bathing needs, medication needs, etc. If she is already in a facility, that place get give the information to the doctor, he can complete it and sign it. If the patient is home bound, the doctor can get the information from the patient and her family.

The doctor should have the forms in his office, but you can also find them online. You are supposed to get an official letter allowing or denying the claim for Medicaid. I would check into that.
