I believe I've been over-billed as there are three identical Claim Numbers in Medicare records.

Exactly! Physician, in this case Hospital, has failed any resolution. The Medicare alert is a Redetermination Request. Where, or to what address, in Medicare do I submit the Redetermination?.

You contact the physician who overbilled you. If that fails, you alert Medicare to a possible fraud.
