We just moved my dad (73 y/o) from an acute rehab center to an Assisted Living Facility after some complications from lumbar spinal surgery 3 months ago.

During his check-in (I was out of the room and didn't witness it) he fell while being transferred from his wheelchair into a weighing apparatus.

The facility filled out an Incident Report, which I read, but refused to give me (or my dad, who is cognitively intact) a copy for our records. Am I (who is his medical/durable Power of Attorney) entitled to this report?

A copy of the incident report goes into his chart but a copy isn't issued to the family.
