I’m the guardian for my sister who is bed ridden from a stroke. Probate Court requires a yearly report. What can the physician and Community nursing home provide me regarding the patient?
I created a summary of my observations and asked for weight and doctor short explanation on progress several times. The GP is not responding to my request and will not advise me date and time she will visit. I met her (1) time after 10 months. She examined my sis for approx. 5 minutes and was out the door. The RN’s who came twice per month were excellent with reports and meeting with me. BCBS approved nurses for 5 months only.
In my area, there aren’t many visiting physicians so I made an in person appointment with a specialist my sister was seeing before the stroke. But that is very time consuming with dressing, ambulance transport etc.
What is your experience with bed bound LO and physician exams?

I think you need to/should have already clarified what reports the court is looking for. Why this is in probate court on a live person is also a mystery.
I would ask the nursing home for their medical records and submit those. In lieu of knowing what else is needed they will show good faith.

In a bed bound senior who is on palliative care there is little done without changes of some sort if blood pressure is regularily measured.
I would submit also to the court your DIARY of attempts to reach the MD and the MD name and number for the court. Let them know you are doing your best.

Remember, a DAILY DIARY kept in ink in a composition book with no tear outs, is admissable in court. While in court clarify exactly what the court requires of you. Take all records that you do have and all phone numbers.

Good luck. Meanwhile daily asking both nursing home and MD to contact you will go a long way; keep cell phone records. Get an ombudsman if you must to negoriate how this will work, but do know you must KNOW what it is you are asking for, and in your question here for us it isn't crystal clear to me.

Does the State want a detailed report or just a record of visits andbhow much you see them? Maybe skme financial statements.
