I don't know about the county component but when I moved my MIL, I called the place I wanted to move her to and they pretty much took care of everything to arrange it. They had a motivation to get her there ASAP so she could start paying them instead of her prior SNF.

Is Medicaid paying for her care? If so, call her caseworker and find out how it will work from County to County. Each County handles their own Medicaid. When hunting for a place for Mom, if I had gone into the next County, I would have had to apply for Medicaid in that County..

If she is private pay, that makes things better. With both instances, you find a new place and talk to them about transferring Mom. Transferring the first of the month. Not sure if there is any pro-rating done especially if on Medicaid.
