My and my moms name is on her house if I die first the house is hers if she dies first the house is mine says she did that for probate

Read the deed carefully. If it says "joint tenants with rights of survivorship" then it passes to you outside of probate. BUT it passes with any outstanding liens still attached to it, such as a mortgage, or home equity or taxes or MERP (Medicaid) which you will have to pay off.
Houses that are left to children in a Will are often sold off to pay the estate debts, because the debts get paid first. You only inherit what is left over.

Would your mom be on Medicaid if she goes into a nursing home? If she is self-paying, nothing changes about what happens to the house.
