There is a very simple form online for IRS. Just fill out and mail in. For Social Security they want a visit or a phone call. I just did this with my brother. They will want to speak to your Mom if she is able to speak. So place the call from her place. My wait time was 55 minutes but they kept saying the serve 55 million people on social security, so guess that makes sense. The social security change works for Medicare as well. If you are her POA you can arrange, by mail, for medicare to send her payment info to you, have her sign the form on the medicare site where you download it. IRS doesn't accept a regular POA but have a form that signed by Mom will allow you to help her file her taxes and sign for her. Basically you are looking at forms forms forms and calls to the SSA or a visit. Don't forget the DMV for change of address and the postal service for mail forwarding and change of address. Basically all my last year has been forms and changing things. It takes a while. So one day at a time.

Call your local SSA. I think this will cover Medicare too. IRS you can change, I think, when u do her taxes for 2019.
