The patient or designated representative must sign an official medical records authorization that is HIPAA compliant and that authorization sent to the medical records department of wherever you want the medical records from. You can be charged 0.75 for each page.

Is this for yourself? If so, just ask your doctor for them or even go to your local hospitals records department.

Is this for someone else?

If this is for a living person, you'll probably need guardianship.

If it's for a dead person for estate and inheritance purposes, you'll need to open an estate and subpoena the records. I'm going through this right now to find out some things about my bio dad. I can subpoena records from wherever necessary to find out some things.

djsales, do you have Medical Power of Attorney? If yes, you should be able to ask for medical records.

If you are changing doctors, the doctor should give you his/her records, but don't be surprised if there is an admin cost for obtaining those records.
