First, you need to talk to his doctor. He has to give an order for rehab. He may be able to have it in his home. Rehab is not permanent. If u want permanent than thats another thing. Look in your local phone book under nursing care. It should list homes in the area. I live in SJ near the Del Memorial Bridge.

Go to the NJ State  website and do a search for rehab centers with his zip code. Another way is to do an internet search for rehab centers in your area - I am sure there will be many but you will have to go through and call them.
Or, contact the Discharge Planning/Social work department at your local hospital as I am sure they have lists as well.
You can also go to the CMS website and search there as well for rehab centers. You can also use the feature on CMS that rates nursing homes to review the centers past surveys and this will help you to determine a baseline of the centers delivery of care. With dialysis it is called "Dialysis Compare" on the CMS website; thus it may be called "Nursing home Compare" but you will have to search the site to find it.
